When making a request for permission, please be sure to include a full citation of the article from which the material will be borrowed (ie, publication name, author and title of article, and year, volume, and page number[s] of publication, as well as the figure and or table number as it appears in the original CHEST journal article). Please state where and when the material will be used, and for what purpose (eg, marketing, commercial, educational, use by author). Please mention any deadlines, and do not forget to include a return address, telephone, and fax number.
