Please read the instructions on the first page of the form and take note of where the form should be filed. Some types of appeals are filed with the municipality or approval authority then forwarded to the OMB; some types of appeals are filed directly with the OMB.

OTHER FORMS

The Board has developed forms for common requests including: adjournments, certified copies of decisions and to summons a witness. Other forms, such as a sample affidavit and acknowledgement of expert’s duty form are also available

*These forms are only to be used for appeals against applications that were submitted to the municipality before January 1, 2007.

FEES

Fees are payable by certified cheque or money order, must be in Canadian funds and made payable to the "Minister of Finance." If paying in person, payment can also be made by cash, debit or credit card. For appeal and other fees see the OMB Fee Schedule.
